7/19/2023 EnchantaVaughn Approved (AM) Upon arrival, client appeared to be in a good mood, as evidenced by his positive spirit. Client was cooperative and compliant in class, while listening to the morning annoucements. Client exhibited he was having a good day. Client displayed positive social behaviors with his class, interacting and participating in all activities. (PM) Client was able to eat lunch without disrupting others. Client was observed getting along well with other group of students during recess. Client exhibited good conversation with authority figures, helping his teacher with delivering something to the office. Client participated fully in group setting, coming up with his ideas on his own. (EOD) Client was witnessed in his seat and following directions. Client struggles with grasping all age appropriate information but tries his best to keep up with the curriculum. Client did not let his hardships in class change his mood and expressed positive feelings toward the end of the day. Client met all objectives, as observed. (AM) QMHP welcomed client to school and provided support and encouragement to ensure a good start to his day and to be on track with achieving his goals and objectives. QMHP monitored client individualized behaviors in collaboration with his teacher and other mental health professional. (PM) QMHP supported client with staying focused in class and reassured him that one on one help is always available. QMHP told client to be patient and reminded him to utilize his resources. (EOD) QMHP praised client for following directions and making good choices. QMHP reminded client that we are all here to help him. QMHP supported client with taking responsibility of his actions and encouraged him to review his goals and objectives everyday. (AM) Client greeted QMHP with a hug. Client verbalized positive feelings about being in school and was excited to see QMHP. Client demonstrated understanding after reviewing goals and objectives. Client displayed high level of participation in class, lunch, and gym settings. (PM) Client responded in a positive manner accepting the positive reinforcement that the QMHP provided by saying "thank you". The client has displayed minor frustration over age-related declining abilities and decided to look at the bright side of things. The client has continued to share his feelings and has been assisted in identifying causes for them. Client explains that he struggles with certain activities in class by wants to keep trying. (EOD) Client was involved in effective learning group and showed interest in 95% of the class activities. Client cheered up and was respectful to his teachers and classmates. Client nodded his head and said I understand. Client realized he was capable of being successful and started working harder.
